If you have been apprehensive about seeing an infertility specialist, we are the clinic for you! We provide a very supportive, relaxing, and caring environment in a highly successful non-hospital setting. Your appointments with us should feel like visiting a friend instead of a medical facility.
The Bay IVF team is very efficient, and the wait for your initial consultation should not be excessively long. We will do our best to see you as soon as possible. Once you have had your initial appointment, your treatment can begin as early as your next menstrual period.
To schedule your consultation with Dr. Polansky, a gynecologist board certified in Reproductive Endocrinology and Infertility, please choose one of the following options:
One of our IVF coordinators will ask you a set of questions related to your reproductive history. It will take approximately 10 minutes to set up the appointment. The Initial Appointment Phone Interview page outlines the questions you may be asked.
If you should call after hours or over the weekend, the Bay IVF answering service will pick up and take a message. One of our staff members will contact you as soon as possible.

Please provide your name and either your phone number or email (or both). You can also leave any message for us.
We will contact you within one business day with a choice of dates and times for your consultation. If you do not hear from us by the end of the next business day, please call us to verify that your request has been received.
After your appointment has been scheduled, Dr. Polansky will review the available information, and you will receive a confirmation email along with a few documents he would like you to become familiar with before your appointment.
Your consultation will focus on a review of causes of your infertility, pelvic ultrasound, semen evaluation, and determination of your probability of a successful treatment outcome.
If you have had IVF treatment and do not have copies of your medical records available, please print out the Medical Release form, complete it, and send it to the clinic(s) where you had your IVF treatment. They will forward your medical records to us. Please note that we should receive these medical records prior to your initial appointment.
